Clinic information
The LHS nurse uses CareDox for electronic charting. Parents benefit by registering your child/children as emails are sent when your child/children receive treatment in the clinic. Also, permission can be given through CareDox for acetaminophen and antihistamines to be administered by clinic staff. To register your child/children email

Acetaminophen is available through the health office. A permission form signed by the parent must be on file. Forms can be picked up in the clinic or downloaded from the school nurse web page.
Over the counter medications can be administered at school. The physician/parent authorization form must be on file and signed by the parent. Medications will be administered according to package instructions. Medications must be in the original container.
Prescription medication can be administered at school. The physician/parent authorization form must be on file and signed by both the physician and parent. Medication will be administered according to the prescription order and must be provided in the original prescription bottle.
Students are not permitted to carry medication at school with the exception of inhalers, epi-pens, and insulin. Permission to carry these three types of medication require a care plan and physician/parent authorization form.
Care plans are available and recommended for students with special health needs.
Vision, hearing, and scoliosis screenings are done every year for tenth grade students within the first sixty days of school. Parents are notified in writing of any abnormal findings. Transfer students are screened at the earliest date possible after school entry. Please call the school if you would like to opt out of the screenings.
